心筋焼灼術後の興奮伝播過程の変化が心筋活動電位持続時間に及ぼす影響の研究

研究概要

カテーテルアブレーション(ABL)前および後1週間のWPW症候群におけるQRST isointegral map(QRST map)およびactivation-recovery interval(ARI)の変化をANOVAを用い比較し体表面上の再分極特性の変化を統計学的に検討した。さらに、12誘導心電図(ECG)およびベクトル心電図(VCG)におけるQRST値のABL前および後1週間の変化についてもANOVAを用い比較検討した。また、イヌを用い興奮伝播過程変化後の体表面上におけるactivation time(AT)とARIの相関関係の変化を検討した。左側副伝導路群のQRST mapではABL前に背部に異常高値領域が右前胸部に異常低値が分布した。右側副伝導路群ではABL前に胸部上方に異常高値領域が右胸部下方に異常低値が分布した。ABL後1週間連日の検討においてはこれらの異常値を呈した領域でのみQRST値は直線的に正常化しABL5日目にABL前値に比し有意差を認めたが他の部位ではQRST値の有意な変化は認められなかった。ECG・VCGにおいても同様な所見が認められた。ABL後における体表面ARIの変化の検討では、QRST mapにおいてABL前に異常値が認められた領域でABL後1日目に比し1週間目にARIは有意に短縮し、これらの部位で活動電位持続時間がABL後徐々に短縮することが示唆された。しかし、ABL前にQRSTの異常値が認められなかった領域のARIは有意な変化を示さなかった。以上のごとく、WPW症候群では体表面上の特定の部位において再分極特性の異常が存在しこの異常は活動電位持続時間が早期興奮部位近傍で延長していることが一因であること、また、この異常はABL後1週間は直線的に正常化することが示唆された。
