同種骨髄移植後の免疫学的寛容に関する因子とその分子機構の解析

研究概要

同種骨髄移植は、造血器悪性疾患、造血器難治性疾患および先天性免疫不全症などに対する根治療法として不可欠である。しかし、移植後に高率に認められる移植片対宿主病(graft-versus-host disease : GVIID)は重症化することも多く、同種骨髄移植の成否を左右する大きな要因となっている。従って、GVIIDの克服は同種骨髄移植の成功率の向上に必須の重要課題と考えられる。GVIIDの発症は、すなわち免疫学的寛容の破綻に起因するものであり、ドナー骨髄に含まれるT細胞が宿主の組織適合抗原を非自己として認識することに端を発する。その結果、種々のサイトカイン産生が促され、宿主の組織適合抗原に反応性を有するT細胞の増殖と活性化が誘導される。T細胞の抗原認識には、T細胞レセプターの他に、種々の接着分子の関与が必要であり、さらなるT細胞の増殖と活性化には種々のサイトカインの関与が要求される。従って、本研究においては、GCIIDと接着分子およびサイトカインの関連性を検討することにより、免疫学的寛容におけるこれらの重要性を確認し、GVIIDの制御につながる方法の確立に資することを目的として解析が行われた。その結果、T細胞の抗原認識に重要なCD28,CTLA4などのcostimulatory moleculesのほかに、ICAMI,VLA4などの接着分子とGVIIDの関連性、さらにIL-1, IL-6, TNFαなどの炎症性サイトカインとIL-4, IL-10, IL-13などの免疫制御性サイトカインのバランスの重要性が明かとなった。
