口腔領域支配神経損傷により発現する神経ペプチドの末梢受容野での分布の解析

研究概要

末梢神経に損傷を与えた場合、神経ペプチドなどの神経調整物質の合成の変化が認められる。1991年に我々は坐骨神経損傷により、一次感覚神経ではほとんど認められないneuropeptide Y (NPY)が後根神経節の中型から大型細胞に発現し、また脊髄後角のIII〜IV層にNPY様免疫活性が増加することを報告した。しかしながら従来は神経節や中枢投射領域での変化に注目され、損傷神経の末梢受容野での変化について報告が無かった。本研究では下歯槽神経と咬筋神経を用い、神経損傷によるNPYの発現について調べた。
1)下歯槽神経損傷による三叉神経節でのNPY陽性細胞の経日的変化
下歯槽神経を切断し、三叉神経節でのNPY陽性細胞の発現を調べたところ、切断3日後にすでにNPY陽性細胞が認められ、その数は切断14日で最大になった。
2)神経損傷により発現するNPYとカルシウム結合蛋白との関係
下歯槽神経切断後、三叉神経節ではNPY陽性細胞の一部はカルシウム結合蛋白のうちcalbindin D28k (CB)陽性であったが、parvalbumin (PV)陰性であった。それに対して、咬筋神経切断では三叉神経中脳路核ではNPY陽性細胞はCB陰性であり、逆にPV陽性であった。
3)下歯槽神経損傷による末梢受容野でのNPYの発現
上頚神経節切除後下歯槽神経に損傷を与えると、歯髄ではNPY陽性神経線維は象牙芽細胞層付近に認められた。切歯舌側歯根膜では太い神経線維に認められ、これらは歯根膜の機械刺激受容器であるRuffini終末であることが免疫電顕で明らかになった。また、歯牙破折でも歯根膜の有髄神経線維や口腔粘膜の無髄神経にNPYが認められた。
以上のことより、神経損傷により発現するNPYは、主に有髄神経線維の再生に関与している可能性が示唆された。
