卵巣癌の組織学的polyclonalityと細胞接着分子の臨床的,基礎的研究

研究概要

【研究目的】日常診療に携わっていると、転移、再発巣の組織像が、原発巣のそれと余りにかけ離れており、その形態のみならず、生物学的特性さえも異り、治療法としての選択に当然の疑問が沸いてくる。【結果】卵巣癌のヌードマウスモデルにおける形態観察、卵巣癌におけるmyc遺伝子の発現、また再発、ならびに剖検症例についてその組織学的、生物学的特性を原発巣からの逸脱として報告した。さらにはラットの実験腫瘍における遺伝子のpolyclonalityについて研究を行い報告した。今回、転移巣、再発巣の腫瘍形態の多様性を細胞の接着分子の関連を卵巣癌の組織学的poly clonalityの解明のため行っている。(1)臨床検討:手術摘出卵巣癌症例(1985-95年)を病理診断の再評価を行った。SLO症例は、摘出標本について初発腫瘍(初回手術摘出標本)との比較検討した。さらに、細胞接着分子としてN-Cadhelinを検討対象をして各特徴的組織像を示す標本を免疫組織化学を行った。(2)基礎的検討:DMBAにて誘発した卵巣癌は腹腔内に播種転移を示し、転移モデルとしての有用性を報告してきた。このモデルについてはすでに組織発生については様々な検討を行ってきたが転移機序、因子についての検討は余り行われていなかった。培養細胞株(DMBA-OC-1)を用いてラット尾静脈ならびに脾臓内移植を行ったが転移腫瘍の形成は認められなかった。(3)睾丸のラット脾臓内移植の組織形態発生についてCadhelinの発現を検討している。(4)新たに樹立した培養細胞株(KOC-4c,-5c,6y)を対象にヌードマウス等への移植実験とCadhelinを原発巣のpolyclonalityについて比較の予定である。96年度日本癌学会で発表した。(5)卵巣腫瘍のMGAE遺伝子について発表した。【今後の展望】腫瘍の形成と接着分子ならびにCTLとの関連についてさらに研究を続けて行きたいと思う。今後集学治療としての可能性まで広げたい。
