サイトカインの遺伝子解析による顎関節内障の病態解明に関する研究

研究概要

本研究では顎関節内で炎症性サイトカインが産生されているかを滑液解析で検討し、さらに関節鏡視下手術の際に得られた滑膜組織から、遺伝子工学的手法によりこれら炎症性サイトカインの遺伝子発現を調べることで、顎関節内病態を細胞レベルで分子生物学的に解析することを目標にした。しかしながら、手術時のバイオプシーによる滑膜の採取による検体の入手は困難であり、この3年間ではサイトカインの遺伝子解析という当初の目的は到達できなかった。そこでわれわれは、近年の顎関節病態形成の仮説である、顎関節内の炎症過程における炎症性サイトカインおよびフリーラジカルの関与についての役割について検討すべく、顎関節内障および変形性顎関節症の患者の滑液解析により顎関節滑液中に存在する各種炎症性サイトカイン、すなわちinterleukin(IL)-1β、IL-6、tumor necrosis factor(TNF)α、IL-8などのサイトカインを同定するとともに、サイトカインと密接に関連するといわれるフリーラジカルの一つであり、強力な炎症反応因子である一酸化窒素(nitric oxide=NO)が、健常人の顎関節滑液中にはほとんど検出されないが、顎関節内障および変形性顎関節症患者において上昇していること、顎関節痛とサイトカインのうちIL-1βが関連があること、またIL-1β濃度とNO濃度は正の相関を認めることを明らかにした。これらの結果から、炎症性サイトカイン、特にIL-1βと一酸化窒素は、顎関節内障の病態形成に関与していることが示唆され、今後これらの遺伝子の検出を行うことで顎関節内障の病態形成のメカニズムが解明される一助となる成果が得られた。
