研究概要

尿マススクリーニング後の本邦と米国の神経芽腫の発生状況と分子生物学的性状検討するため、両国の神経芽腫の細胞株と新鮮腫瘍を対象とした。方法はN-myc遺伝子、p16,DCC,DPC4,MADR2遺伝子を検索し、一部疫学的調査も加え比較検討した。マレーシアの検体は、本年は細胞の収集のみにとどまった。本邦の81例の腫瘍(うちN-myc遺伝子増幅10例)と正常細胞を用いてLOHの検討を行った。LOHは1pでは25%、2qでは32%、9pでは40%、11qでは25%、14qでは32%、18qでは40%に認められた。9番の染色体欠失地図を作成したところ、9pではD9S162とIFNAの間に、9qではD9S152とD9S158の間が高頻度のLOHがみられた。9pのこの領域にp16遺伝子が存在するため、本邦の81例と米国の46例(うちN-myc遺伝子増幅18例)でSouthern法を行ったが、異常はみられなかった。PCR-SSCP法の検討では、本邦の80例中1例にcodon52で変異がみられたが、米国の46例では変異はみられなかった。NBの細胞株のp16遺伝子の発現では、本邦で樹立した10株中6株、米国で樹立した9株中6株で発現の消失がみられ、両者に有意差はみられなかった。Methylationは、本邦の6株中3株と米国で樹立した6株中2株にみられ、NBではmethylationの機構が重要と思われが、両者に有意差はみられなかった。18qでは、本邦例のみDCC遺伝子の発現の検討を行い、細胞株25株中12株(48%)、新鮮腫瘍32例中14例(44%)に発現の消失や減弱がみられた。25個のエクソンでPCR-SSCP法を行い、codon176でとcodon1105でmissense変異をみいだした。本邦の埼玉県と全国統計の疫学調査において、神経芽腫はマススクリーニング後には約2倍に増加しているが、1才以上の進展例は減少していないことを明らかにし、米国とカナダの共同研究の結果もほぼ同様であることが明らかになった。
