心筋細胞のアポトーシス誘導機構と心筋症病態形成に果たす役割に関する研究

研究概要

(1)DBA/2マウスにウイルスを接種することにより惹き起こした心筋炎マウス心臓より、DNAを抽出し、digoxigenin‐dUTPにより3末端標識し、アガロース電気泳動を行い、アポトーシスに特有なDNAラダーの検出を行った。ウイルス接種後3日目の心臓ではDNAラダーは認められないが、5日目よりアポトーシスが出現し、14日目まで明らかなDNAラダーを認めた。定量化したところ、DNAラダーの最大は細胞浸潤、心筋壊死が広範囲で認められる7日目であった。一方、心臓切片標本を作成し、In Situ Apoptosis有無につき検討した。ウイルス接種後5日目よりアポトーシスが出現し、7日目にはその部位は広汎にわたっていた。その後14日目でもアポトーシスが持続していた。また、免疫組織化学によりFas抗原とFasリガンドの発現を調べたところ、両者とも正常心では発現が殆ど見られないのに対して、ウイルス接種後7日目の心臓切片で浸潤細胞の膜表面に強い発現を認めた。この結果、ウイルス感染による心臓細胞死にFas抗原‐Fasリガンドシステムを介したアポトーシスが関与している可能性が示唆された。この時期、ウイルス感染マウス胸腺や脾臓において活発なアポトーシス(DNAラダー)を認めた。さらに、Fas抗原に遺伝子変異がある1pr/1prマウスを用いて同様のウイルス接種実験を行ったところ、心臓内のアポトーシスはDNAラダーで著減する一方、心筋壊死の増加によりかえって死亡率が増加した。これは、免疫応答細胞のアポトーシスを抑制すると、ウイルスの除去がうまくいかず死亡率を高める結果となったことが示唆された。これらの結果は平成8年11月、第69回アメリカ心臓病学会にて発表された。
(2)ウサギの左冠動脈回旋枝を20分間閉塞することにより作成した虚血再潅流モデルにおいてアポトーシスの有無につき検討した。再潅流後1日〜3日目の心臓から抽出したDNAを用いて、明らかなDNAラダーが出現した。また、心臓切片標本をではマクロファージの浸潤部位に核染色にてアポトーシス細胞がが認められ、虚血心筋においても心筋障害に免疫応答細胞によるアポトーシスが関与している可能性が示唆された。
