肺小細胞癌の生物学的特性を利用する末梢血幹細胞移植に関する基礎的臨床的研究

研究概要

1.末梢血幹細胞採取の至適条件の検討と超大量化学療法の臨床試験
現在までに、肺小細胞癌9例(ED:5例、LD:4例)を対象に、化学療法後の造血回復期に末梢幹細胞採取を行った。全症例において移植に必要な末梢血幹細胞が得られ(中央値、CD34陽性細胞:3.2×10^<16>/kg、CFU-GM:6.3×10^5/kg)、更に超大量化学療法施行例全例にて良好な血液学的再構築が得られた。今後、超大量化学療法施行例における長期予後の検討、腫瘍組織の細胞生物学的因子を含めた予後因子の検討を行い、肺小細胞癌における末梢血幹細胞移植併用超大量化学療法の有用性、さらに適応症例についての検討を行う予定である。
2.骨髄、末梢血、および採取末梢血幹細胞における混入腫瘍細胞の検出
当施設にて樹立されたヒト肺小細胞癌細胞株を用いた基礎的検討では、cytokeratin 19 mRNAが8株中4株(50%)で、GRP-receptor mRNAが8株中6例(75%)で、さらに両者いずれかが8例中7例(87.5%)で発現が認められた、cytokeratin 19、GRP-receptorともに正常の末梢血、および骨髄単核細胞では発現を認めないことより、両者の発現をRT-PCRにて検出することにより正常細胞10^<16>に1個の肺小細胞癌腫瘍細胞の混入が検出可能であった.現在、得られた臨床検体を用いて解析中である。これと再発様式との関連を検討することにより自己末梢血幹細胞移植におけるpurgingの必要性の検証、さらに肺小細胞癌治療におけるlate intensificationの有用性の検証が可能と考えられる。
