乳癌の遺伝子診断

研究概要	乳癌切除症例の遺伝子診断を確立するため、広範なヒト染色体領域に分布する150ヶ以上のマイクロサテライトDNA多形マーカーを用いて200症例の乳癌を対象に癌組織における染色体欠失(LOH)の有無を調べ、高頻度なLOHを示した1p、3p、8p、11p、13q、16q、18q、22qの染色体各腕について詳細な染色体欠失地図を作製することにより、乳癌の発症進展に関わる癌抑制遺伝子の存在推定部位を各々2〜9cMの領域に限局化した。各染色体領域のLOhと臨床病理学的因子との相関を調べたところ、1p、17p、のLOHとリンパ節転移、13q、18qのLOHと充実腺管型乳癌、3p、11p、13q、17qのLOHとホルモン受容体消失との間に強い相関が見い出された。また、術後死亡群では17p13.3領域で89%と高頻度なLOHを認めた。これら特定領域の欠失は、乳癌の生物学的特性を規定するとともに予後因子としての有用性を持つもつものと考えられる。また、16qの欠失は非侵潤癌においても高頻度に認められ、この領域に乳癌発生の初期に関わる重要な癌抑制遺伝子の存在が示唆された。術後死亡群において17p13.3を始めとする染色体領域のLOHが生存群に比して高頻度に見られたことから、これらの領域に存在する癌抑制遺伝子の不活性が乳癌の進展にとって重要なステップであることがうかがわれ、手術時における乳癌組織を用いたこれらLOHのDNA診断は術後の予後予測のマーカーとして有用なものと考えられる。今後、術後5年生存を中心とした再発および生命予後と各LOHとの関連をプロスペクティブ研究として検討し、これらマーカーの予後因子としての位置付けを明確にしたい。
