C型肝炎陽性者における肝癌予測因子としての血中MAGE癌退縮抗原の測定

研究概要

C型肝炎ウイルス関連肝癌(hepatitis C virus-associated hepatocellular carcinoma,HCC-C)における血清中MAGE-4タンパク質の濃度(n=45,mean=2.160ng/ml)は健常人対照(0.327ng/ml)と較べて有意(p<0.001)に高値を示した。また、C型肝炎ウイルス関連肝硬変(HCV-associated cirrhosis,LC-C)の場合(n=55,1.072ng/ml)も有意(p<0.0001)に高値を示すことが分かった。
血清中のMAGE-4タンパク質陽性率(健常人での平均値+2SDをcut-off値とした)は、HCC-Cの患者の46.7%(n=45)およびLC-C患者の32.7%(n=55)であったが、B型肝炎ウイルス関連肝癌では0%(n=3)、B型肝炎ウイルス関連肝硬変で6.1%(n=49)、アルコール性肝疾患の8.5%(n-47)、および健常人で2%(n=49)であった。また、MAGE-4抗原はHCG RNAとは相関しなかった。これらの事より、肝再生や線維化、あるいはHCV感染によりMAGE-4抗原が出現した可能性は少なく、HCVによる肝炎や肝硬変の一部で、すでに微少癌が発生している可能性が示唆された。
以上のことより、HCV感染者で、血清中MAGE-4抗原量の推移を見ることは、肝癌への移行を予測する上で重要であると考えられ、今後、MAGE-4の発現例が治療後によりどのような経過をたどるのか、また予後との相関があるのか否かを解析する(Cohort study)必要があると思われる。
