膵β細胞ATP感受性K^+チャネルとSUレセプターの機能協関と糖尿病における異常

研究概要	インスリン非依存型糖尿病(NIDDM)の経口血糖降下剤としてスルフォニルウレア(SU)剤が広く用いられている。しかしながら、SU剤による治療の臨床上の問題点として、長期の服用により糖尿病の悪化とともに血糖降下作用が消失する(二次無効と一般的に呼ばれる)ことが指摘されている。我々はNIDDM状態下での膵β細胞では細胞内ブドウ糖代謝機構に障害が存在する事実を明らかにしてきた。そこでNIDDMでのSU剤の二次無効の発症機序を明らかにする目的で、膵β細胞内にmetabolic unconplerの2,4‐dinitrophenol(DNP)を用いて人為的に代謝異常を導入し、SU剤インスリン分泌促進効果に及ぼす影響を検討するとともに、K_<ATP>チャネル活性抑制効果さらにはSU受容体(SUR)への結合への影響について検討を加えた。5.5mMブドウ糖存在下のインスリン分泌を代表的なSU剤のglibenclamideは濃度依存性に増強したが、100μMDNP依存下では増強効果の消失を認めた。DNP非依存下ではK_<ATP>チャネルは100μMglibenclamideでほぼ完全に抑制された。一方DNP存在下ではチャネル抑制はほとんど認められず、さらに大量のglibenclamideが抑制に必要であった。SURに対する〔^3H〕-glibenclamideの結合能についてはDNP存在下あるいは非存在下において差を認めなかった。以上より膵β細胞内での糖代謝機構の異常によりSU剤によるインスリン分泌促進効果が消失し、この発症機序の少なくとも一つがSU剤のK_<ATP>チャネルの抑制作用の低下によることが明らかとなった。K_<ATP>チャネルはSURとinward rectifier K^+channel(BIR)の二つの蛋白の複合体により形成されていることが知られているので、このチャネル活性に対する抑制効果の低下は、SURとBIRとの間の機能的連関の破綻による事実が明かとなった。
