難治性感染症を繰り返す先天性食細胞機能異常症の解析

研究概要

本研究は2年目を迎え、班員の協力により下記の成果を得た。まず慢性肉芽腫症については新たな患者の発見、病型分類、臨床症状と病型との関係、変異遺伝子の解析、家系、RELP、natural historyなどの調査を行った。患者としては都合210名が把握できた。このうちシトクロム大鎖欠損(X染色体性)患者については、新たに14名の変異遺伝子の解析が進み、都合19名の変異が同定された。また、シトクロム小鎖については、7名の患者の変異が同定された。シトクロム大鎖の欠損が好酸球には見られないCGD患者で解析が進み、プロモーター領域の変異が見いだされた。なおp47欠損については、調べた全員で同一の変異があり、その上流にホットスポットと考えられる部位があることが明らかになっており、またp67欠損については挿入、スプライス異常の2例が解析されている。また、これらのcDNAをレトロウイルスに組み込んだ2種のベクターについて、その発現や制御について検討が進み、本疾患患者の遺伝子治療の適応の可能性について検討を重ねた。好中球が正常な付着能・走化能を欠く白血球粘着不全症、および走化能、活性酸素産生能が低下する細胞内アクチン異常症については、特に異常遺伝子の面から解析を進めた。特に後者については、その原因がβアクチンの異常であることが明らかになり、そのcDNAを分離して遺伝子解析が進み、マウスモデルを作成中である。一方、成熟好中球の数を制御する機構の一つにアポトーシスが考えられるが、好中球におけるアポトーシスの役割について検討された。また接着を制御する蛋白質が見いだされその解析が進んだ。このほか好中球の走化性に一過性の異常のある高IgE症候群の検討が行われた。
