単クローン抗体惹起進行性変病モデルみよる糸球体硬化機序の解析

研究概要

非可逆性硬化性腎病変は雌性Wistarラット(7週令)の左腎摘出後に抗Thy-1単クローン抗体(mAB)1-22-3,500μgを1回静注する事により作製した。また両腎ラットに同様にしてmAb1-22-3を1回静注した可逆性モデルを作成し、種々のパラミターの動態を不可逆性のものと比較検討した。片腎非可逆性硬化性モデルでは、両腎可逆性モデルに比較して早期ED-1,TRPM-3陽性細胞の糸球体への浸潤が始まる傾向が認められたが、ピーク時の浸潤細胞数は両群間で著差は認められなかった。糸球体でのαsmoothe muscle actin(α-SMA)陽性細胞は両群ともmAb1-22-3投与3日後に初めて陽性となり、その出現時期、程度に差は無かった。間質のα-SMA陽性細胞出現頻度は非可逆性群で有意に高かった。糸球体におけるTGM-β,collagen type ・の沈着は、両群とも抗体投与後2週で初めて陽性となり、2週、3週で、その程度に著差は無かった。発症初期の糸球体変病の程度に可逆性、非可逆性両群に著差がない一方で、間質病変が非可逆性群で出現時期が早く程度も強いことからTRPM3陽性の活性化マクロファージ並びにα-SMA陽性細胞の間質への浸潤が間質変病増悪に関与し、変病初期の間質変病の程度が非可逆性の糸球体硬化性変病誘導の重要な因子であると考えられた。また前年度予防的前投与により有効性が証明された紫苓湯は抗体投与1日後、並びに7日後からの投与でも進展制御効果を有することが判明した。また片腎モデルであることからも関心を持たれる血行動態に影響を及ぼす薬剤の効果につき検討を加えた結果、ACE-阻害薬は蛋白尿を減少し(19.2±3.5 vs.150.5±17.1mg/day)、腎糸球体障害、腎間質障害を軽減し、TGF-β,typ ・,・・・ collagenのmRNA、及び蛋白レベルでの発現を抑制した
