セメント質と歯根膜をともなったハイブリッド型人工歯根の開発

研究概要

【研究目的】
現在、歯を喪失した場合の修復法の一つに骨癒着型の人工歯根が広く用いられているが、このシステムは必須の緩衝装置である歯根膜を欠くため早晩、歯槽骨を損傷し、多くは数年で脱落あるいは機能不調に陥る。この点を改善するため自然の歯根膜とセメント質を再生する原理と技術を確立し、さらに歯根膜とセメント質付きの人工歯根を創製する。
【方法】
我々は昨年度までに骨形成サイトカインである骨形成蛋白質(BMP)の抽出方法を確立し、強力な活性BMP(BMPカクテル)を大量に準備した。次に我々が開発検討した8種類以上の細胞支持体のなかでコラーゲン線維膜とBMPと組み合わせた複合体を組織工学的素材として用いた。ビ-グル犬の歯を抜去し、再植する時に歯根膜を、ベルト状(Belt type pefect)と、根尖側2分の1(Apical-half type defect)という2種の規格にしたがって部分削除し、その欠損部にBMP複合コラーゲン線維膜を巻き付けて再植し、経時的に定量的に観察した。削除しない再植例(正常再値例)と削除したがBMPを投与しない例と、2種類の対照をおいた。
【結果と今後の展望】
6週目のapical half型組織学的所見では、残存部と欠損部の境界から有細胞セメント質と歯根膜が再生するのが確認された。しかし、根尖部に近づくにしたがって歯根膜形成よりも骨癒着(Ankylosis)が多くなった。これに対しbelt-typeでは良好な歯根膜とセメント質の再生がみられた。新生セメント質は、切削された象牙質の表面に僅かに露出したコラーゲン線維を足場にして、先ず線維性の層状組織をつくり、次第に石灰化してセメント質を作る。この線維性組織層中のコラーゲン線維が、連続して層外に延び出し、大略、垂直性に走行し歯槽骨にまで達している。今後担体としてのコラーゲン線維膜の幾何学的溝造を改善することによりさらに広範囲の歯根膜とセメント質が再生できる見込みである。
