消化管癌における細胞周期調節因子の遺伝子異常と遺伝子・染色体不安定性との関係

研究概要

1)消化管癌における遺伝子および染色体の不安定性
胃癌について遺伝子不安定性をマイクロサテライト法で検討し、replication error (RER)を高分化腺癌、低分化腺癌では30%に、スキスル癌では約70%に認めた。また、若年者胃癌では、約70%であり、特にBRCA1領域であるD17S855に高頻度であった。食道癌では、6領域について検討し、RERを33%に、loss of heterozygosityを5%に認めた。ミスマッチDNA修復系遺伝子の異常は、まず陽性コントロールとしてHNPCCについて検討し、多くにおいてhMSH2hとMLH1の変異を検出した。胃癌培養株では検索した範囲ではこれらの遺伝子に明らかな変異はなかった。胃癌および前癌性病変についてテロメラーゼ活性とその鋳型RNAであるhTRの発現を検討し、大部分の胃癌および比較的多くの前癌性病変において活性および発現が認められた。
2)消化管癌における細胞周期調節因子の遺伝子異常
胃癌、大腸癌ではcyclin Eの遺伝子増幅、食道癌ではcyclin D1の遺伝子増幅が特異的に認められるが、それぞれに過剰発現も伴っており、癌の進行度と相関していた。cyclin/CDK複合体の最終的活性に関与するCDC25Bの発現は、70%の胃癌で亢進しており、それは悪性度と相関する傾向にあった。CDKインヒビターでは、p21遺伝子の発現とp53遺伝子変異とに胃癌培養株では相関性が認められた。遺伝子変異はなかった。食道癌培養株の多くにp16遺伝子の欠失を認めたが、胃癌では培養株、組織どちらにも異常は殆ど認められなかった。しかし、p16の発現低下は約20%の胃癌に存在し、深達度や進行度と相関していた。これには、プロモーター領域の高メチル化が関与しているものと考えられた。
尚、1)と2)との相関性の検討は次年度の課題とした。
