消化管癌における細胞周期調節因子の遺伝子異常と遺伝子・染色体不安定性との関係

研究概要

1.消化管癌における遺伝子および染色体の不安定性
胃の腸上皮化生における遺伝子不安定性をマイクロサテライト法で検討した。1領域でもreplication error(RER)を示すものは、坦癌症例の腸上皮化生の27%に認められ、全てが不完全型腸上皮化生であった。特に、RER陽性は、癌の周囲に限局しており、また癌と同一パターンを示すものも認められ、遺伝子不安定性に基づいたフィールド癌化の可能性が示唆された。
大腸癌および腺腫についてテロメラーゼ活性を定量化して検討し、中等度異型腺腫の時点から強い活性を示すものが存在し、それは形態学的に核異型と相関することを明らかにした。最近、テロメラーゼの触媒サブユニット(EST2/TRT1)がクローニングされたが、その抗体を用いた免疫組織化学的検討で、胃癌、大腸癌ではそれが核に局在し過剰発現していることを見いだした。
2.消化管癌における細胞周期調節因子の遺伝子異常
胃癌、大腸癌におけるCDKインヒビターp27の発現をmRNAおよび蛋白レベルで検索した。胃癌におけるp27の発現の減弱は、mRNAでは40%、蛋白では60%の症例で認められた。胃癌、大腸癌ともに27の発現の減弱は癌の悪性度(ステージ、深達度、リンパ節転移)と有意な相関性を示した。
3.テロメラーゼ活性と細胞周期調節因子との関係
テロメラーゼ活性とcyclinおよびCDKインヒビターの発現との関係を胃癌において検討し、p21、p27との相関はなかったが、cyclinEの過剰発現と相関する傾向が認められた。多数例について検索し確認する必要がある。
