胃の前癌病変における遺伝子異常の分子病理学的解析

研究概要	1.腸上皮化生を背景粘膜に有するヒト胃癌16例に関して、microdissection法を用いて、癌部ならびに周辺非癌胃粘膜におけるマイクロサテライト解析を行った結果、4例において腸上皮化生部にレプリケーションエラー(RER)を認めた。形態学的にはRER陽性腸上皮化生には中等度以上の異形成が見られた。さらに、通常の生検・手術材料に対するルーチン非放出性RER検出系を確立し検討したところ、胃癌104例中5例(4.8%)、胃腺腫48例中2例(4.2%)において1領域以上でRER陽性であった。これらRER陽性粘膜病変の臨床病理学的特徴は、(1)他の腫瘍性病変の合併頻度が高い:7例中3例は、同一胃内、あるいは他臓器に腫瘍性病変を合併(胃腺腫2例で異型上皮巣の合併、胃癌1例で横行結腸癌合併)していた。(2)他領域にRER陽性を示す胃癌は高齢者の分化型腺癌である:検索した4領域中3領域以上でRER陽性の症例は全て高齢者(60歳以上)の分化型腺癌であった。 2.ヒト胃癌26例の新鮮凍結材料の腫瘍病変部と腸上皮化生部についてテロメラーゼ活性、テロメラーゼRNA (hTR)発現を検索したところ、テロメラーゼ活性は、胃癌26例中23例(88%)に認められ、一方、hTRは26例中21例(81%)で癌部において非癌部に比較して過剰発現を示していた。また,9例では,非癌部胃粘膜においてもテロメラーゼ活性を認め、それらはいずれも腸上皮化生を含んでいた。更に、非癌部粘膜における相対的hTR発現量ならびにテロメラーゼ活性出現頻度の増大に伴って、Helicobactor pylori感染数の増加が認められた。以上、本研究により前癌性病変としての分子病理学的性格を有する腸上皮化生の抽出が可能になったと考えられる。
