疾患単位確定を目指した境界領域的リンパ増殖病変の臨床病理学的分子生物学的研究

研究概要

悪性リンパ腫およびその周辺に位置する種々のリンパ増殖病変は臨床病態のみならず生物学的さらには疫学的に極めて多様性に富む疾患スペクトラムを形成している。特に境界領域的リンパ増殖病変は従来ある程度疾患単位として記載されたT細胞領域異形成や胚中心進展性異形成から、疫学的に極東アジア地域に多い特殊なEpsten-Barr virus(EBV)関連病変、臓器移植後等の免疫不全合併病変、さらに今後本邦での増加が予測される高齢化に付随した免疫機能の低下や自己免疫異常などを背景とするものまで多岐に亘る。しかし、それら病変の認識自体極めて稚拙な段階にあり、真に学際的な研究による整理および本態の解明は急務である。本研究では、悪性リンパ腫から境界領域的病変に亘る疾患スペクトラムの把握と類型化に意を用いた。平成8-9年度の当該期間中にEBVが関与するリンパ増殖病変については新たな病型を見出し、現在学術誌に投稿中である。これらは病勢が比較的急速に進行し、EBVの分子生物学的検索によってのみ腫瘍への移行が示唆されるものであった。しかしながら、増殖細胞に証明されたEBVのclonal integrationが腫瘍性か特異な反応状態かのいづれと見るかは厳密な意味での定義が困難であり、今後の病理総論的な観点からの整理が期待される。一方、高齢者等にしばしば認められ、且つ現時点での分子生物学的手法では腫瘍性格が証明されず境界領域的リンパ増殖病変と判断された症例の経過は一般に緩徐であり、その帰趨は不明であった。当然、その本態の如何は不分明のままであったと云わずるを得ないが、その本質的な意義を解明し得るのは当該研究期間を超えた長期的な臨床病理学的研究の継続のみとも思われた。本研究から得た所感はこれら境界領域病変は各種病因と個体の反応性との相互作用、あるいは異なる遺伝情報間の相互作用における“変容"の過程であるということであり、次年度以降も諸々の悪性リンパ腫亜型と関連を含め研究を続行する予定である。
