サルコイドーシスの慢性化、難治化に関する病態生理学的研究

研究概要	サルコイドーシスの慢性化、難治化についての臨床的課題のひとつとして、予後の指標を明確に見出し、治療を早期に有効に加えるべきかどうかの判断をすることがある。すでに評価してきた指標として発症時年齢、胸部X線上の病期、肺外病変、過去のステロイド投与歴などがある。これらの指標と病勢の指標の中で、病変部位より産生遊離されている活性型ビタミンDに焦点をあてて検討した結果、以下の点が明らかにされた。 1)サルコイドーシス男性症例では血清活性型ビタミンD濃度と血清イオン化カルシウム濃度は有意な正の相関を示したが、対照健常人では負の相関を示した。加えて、血清活性型ビタミンD濃度は肺外病変のある症例では高値を示した。また血清ACE値とも有意な正の相関を示した。これらの相関は、女性症例では認められなかった。女性では、月経周期、妊娠、出産、閉経によるエストロゲンの産生遊離亢進あるいはその欠乏が、ビタミンD、カルシウム代謝に少なからぬ影響を与えることが想定される。したがって、血清活性型ビタミンD濃度により病勢を評価し縦断的な予後の指標として用いるには多様な因子を考慮しなければならない。今回の検討より男性症例では、血清活性型ビタミンD濃度は血清イオン化シウムに大きな影響を及ぼすと同時に、疾患の全体的な広がりを反映していると考えられた。 2)サイコイドーシス症例において、血清活性型ビタミンD濃度の骨代謝への影響は女性症例で、血清活性型ビタミンD濃度と骨密度との逆相関として認められた。男性では認められなかった。年齢でマッチした症例間、健常人との間には骨密度の有意差は認められなかった。活性型ビタミンDが消化管からのカルシウム吸収を亢進させる一方で、破骨細胞を活性化して骨吸収を亢進させている可能性がある。性差に関しては、今後の検討を必要とされる。
