心不全進展におけるエンドセリンの関与の解明に関する分子生物学検討と新治療法開発

研究概要

エンドセリン(ET)-1は、心筋に対して細胞傷害作用や肥大作用を有する。今回、慢性心不全(CHF)における内因性ET-1の病態生理的役割を検討し、拮抗薬の治療薬としての可能性を検討した。ラットの左冠動脈を結紮して心筋梗塞を作成した(CHF群)。偽手術群には開胸手術のみを行った。手術3週後の検討にて、ラットは心不全になっていることが確認された。この時期、左室でのET-1mRNA発現は、偽手術群よりCHF群にて著明に亢進しており、LVの組織ET-1レベル(ペプチドレベル)もCHF群で著明に増加していた。CHF群にETA受容体拮抗薬であるBQ123または生食(Vehicle)を浸透圧ミニポンプにて12週間投与した。生存率は、BQ123投与群の方が生食投与群より著明に高かった(85%vs43%,P<0.01)。生存したラットでの検討にて、BQ123の投与はCHF群の左室+dP/dt maxの低下や左室拡張終期圧の上昇を改善する事が示され、また、病理組織学的検討にて、BQ123の投与はCHF群の心臓リモデリングの進展を抑制することが示された。抗ET-1抗体を用いた免疫組織化学法によるET-1の染色性(ET-1様免疫活性)は、CHF群の心筋細胞にて著明に増大していたが、冠動脈内皮細胞では両群間に違いは認められなかった。以上より、内因性ET-1は心不全の進展に関与しており、ETA受容体拮抗薬が心不全に対する新治療薬になり得る可能性が示唆された。
