血管内皮細胞機能に及ぼす血圧日内変動の影響

研究概要	【目的】高血圧は動脈硬化の危険因子であるが、血圧の動揺性自身が血圧の絶対値と独立して血管障害をおこすかどうか不明である。このこと検討するためsinoaortic denerved(SAD)ラットを用い内皮機能および血管傷害後の内膜新生の程度を調べた。【目的】SADラットは両側上喉頭、大動脈減圧神経、頚部交感神経幹を切断し頚動脈洞を10%フェノールで処理した。血圧は動脈留置カテより覚醒下に観血的に測定した。Mean arterial pressure(MAP)を連続3時間、20秒毎にサンプリングし、平均値(average-MAP)および標準偏差(SD-MAP)を計算した。手術4週後に内皮保存大動脈リング標本をbalansed salt solution(BSS)中にて張力測定し、アセチルコリン(Ach)に対する弛緩反応を検討した。また、BSS中のNOxを測定した。また、フォガティーカテにて総頚動脈を傷害し、2週後に内膜/中膜面積(I/M)比を計算した。【結果】average-MAPの平均はSAD群で127±3.4mmHg、Sham群で120±5.9mmHgと有意差はなかったがSD-MAPの平均はSAD群で13.8±1.2mmHg、Sham群で3.7±0.3mmHgと有意にSAD群で血圧動揺性が大きかった。Achによる弛緩はSAD群で有意に低下していた。またBSS中のNOxは有意に(SAD 17.7±4.4pmol/mg,Sham 31.3±1.9pmol/mg)SAD群で低値であった。ニトロプルシッドの弛緩反応に有意差はなかった。I/M比はSAD群で有意に高値であった(SAD 1.36±0.15,Sham 0.86±0.13)。【総括】血圧動揺性の増加は、血圧の絶対値と関係なく内皮機能を障害し、動脈硬化の進展に促進的に関与することが示唆された。
