腰椎椎間板ヘルニアによる神経因性疼痛発現機序の解明

研究概要	本研究の目的は腰椎椎間板ヘルニアの神経因性疼痛発現機序を解明する一助とすることにある。平成9年度は(1)ラットを用いて硬膜外神経根上に自家椎間板組織移植による神経根内に産生される炎症性化学物質と痛覚過敏を指標とした疼痛行動出現との関連性、(2)ヒト椎間板ヘルニアでみられる機械的圧迫がこのモデルにおける痛覚過敏に及ぼす影響を観察、検討した。ラットを用いて尾椎より椎間板組織を摘出し、髄核移植を神経根上に行ない平成8年度に確立した神経根性疼痛モデルを作成した。経時的に神経根内のサイトカインであるインターロイキン-1β、インターロイキン6、Phospholipase A_2、一酸化窒素合成酵素をreverse transcription-polymerase chain reaction (RT-PCR)を用いて観察した。圧刺激に対する痛覚過敏は処置後3日から3週間、髄核移植群で認められた。RT-PCRでは処置後一週で神経根、後根神経節でインターロイキン-1β、インターロイキン6、Phospholipase A_2、一酸化窒素合成酵素のmRNAの発現がみられたが、2週以降ではインターロイキン-1β、Phospholipase A_2、一酸化窒素合成酵素の発現は見られなかった。インターロイキン6は処置後4週でも発現が認められた。絹糸による神経根機械的圧迫をこのモデルに加えることで圧刺激に対する痛覚過敏は消失し、熱刺激に対する痛覚過敏が処置後1週まで有意に観察された。神経根の組織学的検討では神経根結紮のみ(痛覚過敏発現なし)と神経根結紮し髄核を留置したものとの間には小径有髄線維の増加が見られるものの差異はなかった。損傷神経根内で産生されるこれらの炎症性産物は痛覚過敏、すなわち神経根性疼痛の発現早期に関与する可能性がある。また、組織学的変化と関係なく異なる痛覚過敏が発現したことから痛覚過敏の維持には脊髄を含む中枢機序が関与する可能性がある。
