血管作動性物質の肝血管収縮部位の同定 肝血管閉塞法の確立

研究概要

我々は新たな肝毛細管圧の測定法の確立を目標としてきた。本年度は前年度に確立した肝動脈と門脈の両方から潅流する肝潅流標本と肝毛細管圧測定法を用いて血管作動性物質の肝血行動態に及ぼす作用を検討した。Triple occlusion pressure (Pto)より肝毛細管圧を測定し、肝血管抵抗を門脈抵抗、肝動脈抵抗さらに後毛細血管抵抗である肝静脈抵抗に分けてヒスタミン、アセチルコリン、ノルエピネフリン、血小板活性化因子の血管作動性物質の肝血管収縮部位の局在を明らかにした。
その結果、ヒスタミンは容量依存性に門脈抵抗と肝静脈抵抗を増加させたが、肝静脈抵抗の増加が有意に大であった。また、肝動脈抵抗の変化は認められなかった。肝流出側抵抗の増大に伴って肝重量は増加し鬱血を来した。一方、ノルエピネフリンは肝動脈抵抗及び門脈抵抗の増加が肝静脈抵抗の増加に比べて著明であり、前毛細血管を優位に収縮させた。肝重量は流入側の血管抵抗の増大により減少した。また、アセチルコリンと血小板活性化因子は肝静脈の抵抗を同程度に増加させた。その結果、肝重量はアセチルコリンでは有意な変化を示さなかった。
このようにイヌの肝臓では血管作動性物質により肝臓内血管収縮部位が異なることが我々が開発したTriple occlusion法により判明した。
