ヒト妊娠・分娩が骨量に及ぼす影響に関する臨床的研究-type I collagen代謝動態の観察を主眼として

研究概要

I Type I collagen関連骨代謝マーカーの臨床的有用性の検討-骨塩量変動との相関について
生理的に大きな変化が連続する妊娠-分娩-産褥期の骨代謝状況を理解するためには、静的な指標である骨量のみでなく、骨代謝状況をreal timeで反映する簡便なbiomarkerの動態観察が要求される。そこで、近年開発されたType 1 collagen生成markerのP1CP(carboxy-terminal propeptide of type 1 procollagen)と分解markerのICTP(pyridinoline cross-linked carboxy-terminal telepeptide of type I collagen)の臨床的有用性を検討した。閉経後3年以上を経過した健康な婦人77名を対象とした前方視的研究の結果、(1)血中type 1 collagen関連マーカー,とくにICTP値はDXA法で測定した腰椎(L2-4)の骨量増減と良好に連関し、PICP,ICTP値測定により、生体内骨代謝状態のリアルタイムの観察が可能であることが確認された。
II 妊娠・分娩が骨量に与える影響:骨量および血中Type I collagen関連マーカー測定による観察
妊娠・産褥期の母体に特異的な骨代謝状況の解明を目的として正期産例を対象に、分娩周辺期より分娩後9カ月までの血中骨代謝マーカー値と腰椎(L2-4)の骨量を測定した。その結果、以下の知見を得た。
(1)母体の骨代謝は妊娠時より分娩後まで亢進状態にあるが、骨形成がほぼ一定であるのに対し、骨吸収は分娩後に急激な一過性亢進を示す。
(2)そのため、授乳の有無、および授乳期間にかかわらず、母体の骨量は分娩後3ヵ月頃では有意に減少する。また、授乳中は骨量は持続的に減少し、この減少は月経の再来後4-5ヶ月で回復する。従って長期授乳者では骨量減少は著しい。
(3)血中骨代謝マーカー値からの推定では、満期胎児の骨代謝状態は骨形成、骨吸収ともに母体の数十倍のレベルにある。
