婦人科腫瘍のリンパ節微小転移の遺伝子診断

研究概要

リンパ節転移は婦人科腫瘍の重要な予後因子の一つである.本研究の目的は、腫瘍細胞では発現しているが正常のリンパ節細胞では発現していないmRNA(ここではサイトケラチン(CK)20)をターゲットとしたRT-PCRを行うことによってこのCK20mRNAを特異的に増幅し腫瘍細胞の存在(微小転移)を検出することである.試料として手術時に摘出した腫瘍組織およびリンパ節を用いた.試料よりtotalRNAを抽出し、reverse transcriptaseを用いてcDNAを合成し、プライマーを用いてPCRを行い、その産物を2%アガロースゲルで解析した.本法の検出感度は腫瘍細胞1個/リンパ節細胞10^5個であった.腫瘍組織でのCK20mRNAの検出は、1st PCRでは47% 2nd(heminested)PCRでは100%であった.2nd PCRの結果、子宮頚癌18例、子宮体癌10例、卵巣癌9例の合計37例すべてにCK20mRNAの発現が認められた.リンパ節でのCK20mRNAの発現を組織学的検索の結果と比較すると、組織学的に転移陽性と診断されたリンパ節5個はすべてCK20RT-PCR陽性であった.一方組織学的に転移陰性と診断されたリンパ節についてみると、子宮頚癌14例より採取された89個中2個、子宮体癌8例より採取された76個中2個がCK20RT-PCR陽性と判定された.これらのリンパ節には組織学的検索では検出できない微小転移が存在するものと思われる.以上の結果より、CK20RT-PCRは通常の組織学的検索よりも感度が高いと考えられた.
