研究概要

1.目的: 中米胃癌のリスクファクターを組織型別に明らかにすることを目的とする。
2.方法: コスタリカ、グアテマラそれぞれにおいて、症例対照研究の対象者に面接調査および血清、病理組織の採取を実施。
3.結果
i)症例収集状況
1998年2月までにグアテマラで症例105名、対照118名、コスタリカで症例153名、対照151名であった。
ii)血清収集状況
グアテマラにおいて、症例群105例、対照群106例、コスタリカでは、症例群148例、対照群151例であった。
iii)H.pylori陽性率
グアテマラでは、症例群でのH.pylori抗体陽性率は、71.1%(54/76)、対照群では、75.4%(52/69)であった。コスタリカでは、症例群73.2%(90/123)対照群79.0%(94/119)であった。
iv)Pepsinogen I/II
3.0≦PG I/IIの者に対し、PG I/II<3.0の者のリスクはグアテマラで、1.69(0.86-3.31)、コスタリカで、4.24(2.28-7.88)であった。グアテマラでは、慢性萎縮性胃炎(PG I/II<3.0の者)による胃がんの相対危険度の上昇は顕著でなかったが、コスタリカでは、4.2倍と明らかに上昇していた。
