胃ECLカルチノイド腸瘍の増殖・転移におけるガストリンの役割についての研究

研究概要

ゾリンジャー・エリソン(ZE)症侯群やA型胃炎など高ガストリン血症を呈する疾患の胃にECLカルチノイド腫瘍が生じる事実は良く知られている。本研究ではこうしたガストリンによる腫瘍の増殖や転移促進の細胞内機序について検討した。(1)マストミス・マウスの胃壁細胞、ECL細胞にガストリンを投与したところ、ECL細胞でのみRas、MAPKの活性化、c-fos発現の増強が認められた。一方マストミスECLカルチノイド細胞では、これらに加えてガストリンによりVEGF,MMP2,9の発現の増強を生じた。(2)マストミスのin vivoにガストリン受容体拮抗剤を投与すると、カルチノイドの発生は抑制されるものの、時期とともに発生してきた。逆にガストリン投与は本腫瘍の転移を促進した。一方、正常ラットにプロトンポンプ阻害剤を投与すると、胃カルチノイド腫瘍が発生するが、決して転移は見られなかった。(3)ヒト胃内分泌細胞癌株ECC10と12のうち、10のみがガストリン受容体を有し、ECC10のみでガストリンによる細胞増殖が観察された。本細胞ではやはりガストリンによってVEGF,MMP2,9発現の増強が認められ、さらに細胞膜のラツフリング、アクチン・ストレス線維の形成が認められた。
以上、同じガストリン受容体をもつ細胞であってもガストリンによって細胞増殖のシグナルが活性化される細胞(ECL細胞、内分泌細胞癌など)と、されない細胞(壁細胞)が存在することが明らかとなった。一方転移や浸潤に関係すると考えられるVEGFやMMP2,9の発現はラットのECLカルチノイドでは見られず転移も認められなかった。逆にマストミスのカルチノイド腫瘍やECC10細胞ではガストリンによってこれらの発現が増強され、かつマストミスでは転移が促進された。これらの事実より、ガストリンは胃カルチノイド腫瘍を増殖させるものの、単独では転移浸潤に関与しないと考えられた。
