研究概要

消化器癌における遺伝子診断の臨床応用および遺伝子治療の基礎的研究として、本年度、以下の成果を得た。
I.DNA修復異常の遺伝子診断
(1)ミスマッチ修復活性の検出系の開発
ミスマッチ修復系は抗癌剤がもたらすDNA障害に深く関与していると考えられる。我々は正常組織と癌組織を異なった蛍光ラベルのプライマーを用いてPCR反応を行い、シーケンサーで解析する正確で客観的なマイクロサテライト不安定性(MSI)解析法を構築した。
(2)消化器癌におけるミスマッチ修復異常
上記検出系を用い、食道癌46例中6例,胃癌73例中13例のMSI陽性症例を認め、消化器癌の発生にミスマッチ修復異常の関与が示唆された。胃癌では低分化な癌に有意にMSIが多く、癌の悪性度とも関係していると考えられた。また、胃の正常組織におけるマイクロサテライト配列の長さを各個人間で比較すると、CA repeat長が長い個人において高い割合でMSIを示す傾向が認められた。従って、MSI解析を行う場合は、適切なmarkerの選択と解析する集団における平均的なCA rereatの長さを考慮する必要があると考えられた。
II.肝細胞癌を用いた遺伝子治療の基礎的研究
(1)HSVのtkをexpression vectorに挿入し、このプラスミドべクターをHMG-1 (High Mobility Group 1) と結合させ、ボルテックス法と超音波処理によりポソームに取り込ませ、DNA-リポソーム複合体を形成させた。これを紫外線照射したHVJと反応させHVJ-リポソームを得た。(2)動物肝癌モデルへの遺伝子治療:DNA-リポソーム複合体を、肝癌細胞株HepG2を皮下注して得られた肝癌モデルの片方の腫瘍に100μlずつDirect injectionを行った。翌日より、腹腔内にGancyclovirを注入し、腫瘍のサイズを観察したところ、治療群では 腫瘍サイズの増大は認めなかったが、非治療群では腫瘍サイズの増大を認めた。HVJ-リポソームは、繰り返し投与可能なベクターであり、遺伝子治療の有用なベクターとなることが示唆された。データ採集についての患者本人の同意は得ている。
