研究概要	高ホモシステイン血症に関与する遺伝子変異が家族性高コレステロール血症(FH)における冠動脈硬化症の発症にどのように関与するかを検討した.血漿ホモシステイン値を上昇させるMTHFR遺伝子が制限酵素HinfIにより切断される場合,677番目の塩基がCからTに変化していることを示唆する.正常対立遺伝子をA、異常対立遺伝子をVとすると、遺伝子型はAA、AV、VVの3群に分けられた。血漿ホモシステイン値を測定し得た37例では,AAが11.3、AVが10.8、VVが18.1と3群間に有意差を認めた。男性ヘテロFHにおいてMTHFR遺伝子変異は血漿ホモシステインの高値を介して冠動脈硬化症の発症を促進すると考えられた。白色人種FHではLDLレセプター遺伝子異常以外にFDB(apoB-3500(GtoA)変異)が高頻度に認められる.FDBには3500以外に3531などの変異の報告されている。今回はDGGE法を用いてコドン3500以外の変異の可能性を含め検討した.FH160家系200名,家族性複合型高脂血症35家系51名を含む原発性高コレステロール血症患者405例(男性190例,助成215例)を対象とした.今回のスクリーニングでもapoB変異は認められなかった. MTP欠損症(無βリポ蛋白血症)患者空腸生検から得たRNAからRT-PCRにて全長MTPcDNAを得た。発端者はイントロン9スプライスアクセプターG(-1)-to-A変異のホモ接合体であった。HinfI制限酵素によるPCR-RFLPの結果、母、妹、母方祖父は同変異のヘテロ接合体であったが、父および父方祖父母は正常型であった。STR多型マーカー解析でMTP遺伝子(4q22-24)を含む患者染色体4q21-35の領域は、母由来の染色体のみから成るイソダイソミ-であった。父子判定を8つの異なる染色体にある多型で検討し、父権肯定確率は0.993であった。その結果、uniparental disomy(UPD)という希な遺伝形式によりことが判明した。
