研究概要

最もcommonな成人病(生活習慣病とも呼ばれる)の糖尿病、高脂血症、高血圧、動脈硬化等の発症には過栄養に基づく肥満即ち脂肪組織の過剰蓄積が大きな基盤となっているが、私達のこれまでの研究では上記の多彩な病態は、実は腹腔内内蔵脂肪(とくに腸間膜脂肪)の蓄積が大きな要因となっていることを明らかにしてきた。従って本研究では、脂肪組織を構成する脂肪細胞の分子生物学的特性を部位別(皮下脂肪と内蔵脂肪)に明らかにすることによって、内蔵脂肪の蓄積が多くの病態を発症させる分子機構を明らかにすることが目的である。今年度までに、まず脂肪細胞の遺伝子マッピングにより分子特性を明らかにした。それによると脂肪細胞に存在する遺伝子の約20%が、特に内蔵脂肪では約30%が分泌蛋白をコードする遺伝子であり、その多くはサイトカイン、増殖因子、補体等の生理活性物質である。その中で、血栓形成と関連するPlasminogen activator inhibitor-1(PAI-1)は内蔵脂肪の蓄積と共にその合成分泌が亢進し、内蔵脂肪蓄積で見られる血管合併症の成因のひとつであることが明らかとなった。また今回の研究で、脂肪細胞特異的に発現している遺伝子apMl(adipose most abundant gene transcript-1)を見出したが、その遺伝子産物のうちコラーゲン様蛋白(adiponectinと命名した)は、平滑筋細胞増殖を強く抑制することが明らかとなった。さらに測定系の開発により、このadiponectinは肥満、特に内蔵脂肪の蓄積によって血中レベルが減少し、虚血性心疾患患者でも低下していることを明らかにした。栄養状態に応じて活発に増減する脂肪組織が上記のような生理活性物質の分泌臓器であることを明らかにしてきたが、さらに脂肪細胞で合成、分泌される既知及び未知の物質の同定を行い、その過剰及び欠乏が多彩な病態発症のメカニズムに関与する事実を証明してゆく予定である。
