研究概要	潜在性自己免疫性甲状腺炎の診断に血中抗甲状腺マイクロゾーム抗体測定が有用であることを確立した。そして妊娠初期にこの抗マイクロゾーム抗体をスクリーニング検査し、出産後甲状腺機能異常の発生を予期的に観察した。その結果、抗体陽性者の62%に種々の甲状腺機能異常が発生することを明らかにした。さらに抗甲状腺マイクロゾーム抗体陽性者を対象に高感度法で抗TSH受容体を測定し、出産後バセドウ病発症との関連を検索したところ、受容体抗体陰性者からはバセドウ病の発症はみられなかったが、本抗体陽性者の70%に出産後バセドウ病発症がみられた。バセドウ病の出産後発症は一般婦人の0.54%(出産後婦人の200人に1人)に発症することを明らかにした。慢性関節リウマチの出産後発症を予測するため、妊娠初期に2種のリウマチ因子(抗ヒトIgGおよび抗ウサギIgG)を測定し、出産後発病との関係を検索したところ、両抗体とも陽性者の22%に慢性関節リウマチの発病がみられた。出産後婦人の慢性関節リウマチ発症の頻度は一般婦人の0.08%となった。出産後自己免疫性心筋炎の新しい概念を確立するため、出産後発症した急性心不全患者を検索した。対象とした3症例ともに蛍光抗体法による抗心筋抗体が陽性であり、ELISAによる抗体ミオシン抗体も一例に認められた。出産後自己免疫性心筋炎が存在すること、さらに将来的には抗心筋抗体測定にて発症予測ができることが示唆された。出産後自己免疫性肝炎の概念を確立するため、抗肝臓抗体測定法の確立を試みた。リコンビナントP450 2D6抗原を無細胞系で作成し、Rasioligand Assayを確立した。この方法を用い自己免疫性肝炎のスクリーニングを行うと共に、出産後自己免疫性肝炎なる新症候群の確立が今後期待される。
