自己免疫疾患の出産後発症予測とその予防に関する研究

研究概要	研究は予定通り順調に進行している。血中抗甲状腺マイクロゾーム抗体測定により潜在性自己免疫性甲状腺炎の診断が可能となり、妊娠初期スクリーニング検査から、抗体陽性者の62%に種々の出産後甲状腺機能異常が発生することを明らかにした。さらにこの抗体陽性者で高感度法で抗TSH受容体(TSAb)を測定することにより、出産後バセドウ病発症予測が可能となった。またバセドウ病の出産後発症は一般婦人の0.54%(出産後婦人の200人に1人)に発症することを明らかにした。出産後婦人の慢性関節リウマチ発症の頻度は一般婦人の0.08%であることが明らかになった。また慢性関節リウマチの出産後発症に、妊娠初期における2種のリウマチ因子(抗ヒトIgGおよび抗ウサギIgG)の測定が有用であることも明らかとなった。出産後自己免疫性心筋炎の新しい概念を確立するため、出産後発症した急性心不全患者を検索し、蛍光抗体法による抗心筋抗体、ELISAによる抗ミオシン抗体測定から、出産後自己免疫性心筋炎が存在することが明らかになった。出産後自己免疫性肝炎の概念を確立するため、抗肝臓抗体測定法の確立を試みた。リコンビナントP450 2D6 抗原を無細胞系で作成し、Radioligand Assayを確立した。出産後自己免疫疾患の発症機作として、妊婦の末梢血中リンパ球サブセットの解析により、出産後早期(2〜4カ用にはThlを、後期(6〜12カ月)にはTh2を中心にした免疫リバウンドが起こることを明らかにした。
