研究概要	本年度は、遺伝子治療用ストラテジーの開発を主に行った。再狭窄に対してヒト肝細胞増殖因子(HGF)遺伝子を用いた内皮再生療法の検討を行い、1)HGFの過剰産生により血管内皮の再生が促されること、2)内皮再生に伴い、血管壁でのNO産生が増加していること、3)HGF遺伝子導入血管はアセチルコリンに対する血管拡張反応が回復していること、4)内皮再生により新生内膜が有意に抑制されること、5)内膜抑制は単回投与後8週においても見られること、などを明らかにした。また、バルーン傷害部位では内因性のHGFが減少しており、HGF遺伝子による内皮再生療法は補充療法であり、病態生理学的に見て最も適していることを示した。また、がん抑制遺伝子であるsdi-1遺伝子の導入によりベイングラフト後再狭窄が有意に抑制されること、そしてその抑制は平滑筋細胞増殖抑制のみでなく、細胞周期制御による成人型への再分化及びbaxの過剰発現による細胞死・アポトーシスが関与していることを報告した(Hypertension 1998 ; Gene Therapy 1998)。又、心筋梗塞に対してもHGF遺伝子による血管新生療法が有効であることを明らかにしつつある。更に、サイトカインや接着因子の発現制御を行う転写調節因子NFkBに対するデコイ投与による心筋梗塞の予防を報告した(Nature Medicine 1997)。現在そのメカニズムを詳細に検討しているが、虚血による内皮のアポトーシスにNFkBが関与していることを見いだしており、更にその分子機構について検討を進めている。
