研究概要

当教室で、世界で最初に分離したバンコマイシン耐性MRSA(VRSA)株Mu50,および、その前駆細胞株であるヘテロ耐性株Mu3(hetero-VRSA)の、耐性メカニズムを研究し、これらの株がバンコマイシンの作用点である細胞壁ムレインモノマーのD-alanyl-D-alanineを過剰に産生していることを見いだした。このことは、細胞壁にバンコマイシンが結合消費されることにより、本来のバンコマイシン作用点である、細胞質膜上のlipidIIに到達しにくくなることを示唆した。この耐性メカニズムをaffinity trappingによる耐性獲得と命名した。
日本全国のVRSA、ヘテロVRSAのスクリーニングを細胞集団解析法を用いて行い、大学病院では、すべてのMRSAの9.3%に、非大学病院で、比較的小規模の病院、クリニックでは1.3%にヘテロVRSAが見いだした。VRSAは、現在までに全世界で5株(日本、1;アメリカ、3;フランス、1)のみであるが、ヘテロVRSAがバンコマイシンで選択されると高頻度(百万個に1個)にVRSAが生じることから、すでに全国の病院に、バンコマイシンの治療効果が期待しにくいMRSA株が蔓延していることが推測された。
VRSA、ヘテロVRSAの治療法としては、スルバクタム/アンピシリンとアルベカシンの併用療法を確立した。しかし、新規抗MRSA薬であるテイコプラニンの抗菌力は、VRSA、ヘテロVRSAに対してはバンコマイシンに劣ることがわかり、緊急なVRSA治療薬の開発が今後の課題である。
