疲労感の分子メカニズムの解明

研究概要	疲労および疲労感は、高度に発展した現代社会に生きる多数の人々が日常向き合っている現象であり、人類普遍的な重要な問題である。しかし、その分子・神経メカニズムについては、マラソン後の脳脊髄液中のセロトニンおよびその代謝物の増加が知られている程度で、研究が進んでいない。我々は、慢性疲労症候群(CFS)に対する研究成果より、ウイルス感染、社会心理的ストレス→免疫・内分泌ホメオスタシスの悪循環→免疫抑制サイトカインの上昇(TGFβなど)→ニューロステロイドの低下→脳局所のアシルカルニチン代謝異常→ミトコンドリア障害→グルタミ酸・GABA(γ-アミノ酪産)などの主要アミノ酸ニューロトランスミッターの低下継続→慢性疲労感という疲労に関する作業仮説を提唱しており、この仮説を実証・肉付けし、疲労一般の研究に適用すれば、世界に先駆けてこの分野をリードすることが可能である。本研究では、仮説の根幹をなしている「CFS患者では脳におけるアシルカルニチン代謝異常が存在するのか否か」について、CFS患者と健常対象者とを対象に局所脳血液量と局所脳アセチルカルニチン(ACC)取り込みを検討した。コンピューター上の標準脳モデルを用いた群間比較では、局所脳ACC取り込みは自律神経系の調節や情動などに深く関連しているBrodmann24野と意欲やコミュニケーションにおいて重要なBrodmann9野においてCFS群で有意に減少していた。さらに、マウスに[2-^<14>C]ACCを投与し、脳代謝物解析を行ったところ、ACCのアセチル基は脳において興奮性の神経伝達物質(グルタミン酸、アスパラギン酸)の合成に利用されていた。したがって、CFS患者で認められるACC取り込み異常は、同部位における神経伝達物質の合成低下として、臨床病態と関連している可能性が示唆され、疲労および疲労感の分子・神経メカニズムについての解明に向けての基盤となる成績が確認された。
