肝性脳症における脳高次機能と脳イメージング

研究概要

これまでの肝性脳症に関するわれわれの研究成果を背景として,脳高次機能の解析に加えて脳機能イメージングという新たなアプローチを試み,肝性脳症の早期診断や治療に役立てようとした。平成10年度の本研究によって得られた新しい知見を以下に箇条書する。
1. 肝硬変(代償性、非代償性)例のコンピュータ化した定量的神経心理学的検査法(IBM Thinkpad 760C Type,Otsuka/Kokuyo)の5検査(8評価項目)から潜在性脳症を検出した。
2. 肝硬変例において、MR画像から大脳(前頭葉、海馬、側頭葉)萎縮と大脳基底核領域のT1高信号を検出した。
3. 肝硬変例において、^<99m>Tc-ECD SPECT(PatlakプロットとLassen補正)により海馬、基底核、前頭葉皮質の脳血流量(ml/100g/分)を測定した。
4. ^1H-MRSにより海馬、基底核、側頭葉におけるコリン、グルタミン(グルタミン酸を含む)、ミオ-イノシトール、N-アセチルアスバラギン酸などの濃度を測定した。
