遺伝子導入による肝再生促進療法の確立

研究概要

肝臓の増殖因子を制御することにより有効な肝再生促進療法が開発されたなら広範な肝切除が可能になり根治術の幅を広げることができる。 肝臓には現在、TGF-βやActivin-Aなどのautocrine増殖抑制因子が存在することが知られている。我々はActivin-Aの結合蛋白でその作用をブロックするフォリスタチンの投与によりDNA合成を早め肝再生が促進されることを報告してきた。TGF-βやActivin-Aのリセプターは細胞内ドメインにセリン、スレオニン活性を有し、それぞれ1型キナーゼと2型のサブユニットからなっている。リガンドはII型受容体に結合し、それが1型キナーゼを燐酸化して活性化される。
本研究はこのTGF-βやActivin-Aのリセプターの変異受容体遺伝子を作成しこれをウイルスベクターを用いて肝臓に導入し過剰発現させることにより、すなわち、autocrine増殖抑制作用を内因的にブロックすることにより肝再生を促進させることを目的としている。我々はこのActivin-A・II型変異受容体遺伝子を組み込んだウイルスベクターの作成を既に完了しヒト胎児腎由来細胞株(293細胞)で増殖させている。実際に、培養腎細胞にこのウイルスベクターを投与すると一層の培養細胞が分化して管腔を形成することが認められている。更に、このウイルスベクター1.66x10^6PFUをラットの門脈内に投与して肝細胞の増殖が促進されることの検討を始めた。コントロールとしてE1A,E1B欠失型アデノウイルスベクターにlacZ遺伝子を組み込み、これを293細胞内で増殖させVirusl1.66x10^6PFUを門脈内にone shotで投与し細胞内にβ-galactosidaseを過剰に発現させ、肝組織片を5-ブロモ-4-クロロ-3-インドリル-ベータ-D-ガラクトシド(X-Gal)で染色しβ-galactosidaseの活性度を検討しアデノウイルスベクターの遺伝子組み込み度を検討している。また、今年度は予備実験としてフォリスタチンを無処置ラットの門脈内に投与する実験を行ったが無傷ラット肝臓においてフォリスタチンの門脈内投与により投与3時間後からDNA合成が開始され、それが48時間後にピークを迎えることが確認された。この実験結果は今年のHepatologyに掲載予定である。
