軟骨基質分解産物による軟骨、滑膜細胞代謝に対する影響-サイトカイン産生に関わる転写因子について-

研究概要

関節外科領域では関節リウマチを初めとする関節炎疾患に対する治療法の一つとして関節洗浄が臨床的に有効であるとされています。細胞外基質とそこにある細胞との関連が関節症疾患の慢性化の鍵を握ると考えました。細胞外基質またはその構成成分の状態が細胞に影響を与え、又は影響を修飾する事によって疾患の慢性化さらなる基質破壊が起こる機序を証明することを目的として本研究は行われました。
本研究で我々は
(1) 各種関節炎で炎症性サイトカインによってコラゲナーゼ(MMP-1)、ストロメライシン(MMP-3)等軟骨分解系酵素が多量に産生されている事、関節基質分解産物を初めとする代謝物は関節症の進行を反映しうる事等を明らかにした(石黒ら、1999)。
(2) 転写因子NFκ-Bが骨芽細胞でTNF刺激により活性化、IL-6,IL-8,産生を招来する事が明らかとした。これは関節炎に伴う骨破壊に破骨細胞のみならず骨芽細胞も関与する可能性を示した(黒河内 1998)。(3) 転写因子NFκ-Bが軟骨細胞、滑膜細胞でTNF刺激により活性化機構が高分子ヒアルロン酸の添加によって影響を受ける事を明らかとした。 一方フィブロネクチンの分解産物では転写因子NFκ-Bの活性化を伴う炎症性サイトカインの産生は見られなかった。
(4) 関節炎に関わる新たな遺伝子を検索するためにdiferential display法による検討を行いラットATPaseをクローニングした。
(5) タイプ1コラーゲン合成で転写以後の制御機構の存在とこれがVit Dの影響を受けること、また異常コラーゲン合成下では分子シャペロンの働きが見られることを明らかとした。
本研究を通じて細胞外基質とその分子量の変化が細胞の代謝に影響を与えることを分子生物学的に証明したことは今後の変形性関節症を始めとする慢性関節炎関節症の研究ににとって大きな可能性を提供したと思います。
