NO消去剤の抗ショック治療への応用

研究概要	我々はこれまでの研究により、エンドトキシンショックにおいて過剰に産生されるNOが、遷延性の低血圧に直接関与していること、さらにエンドトキシンショックモデルにNO消去剤(PTIO)を投与すると、抗ショック作用が認められることを明らかにしてきた。一方NOは、組織障害性と保護作用という2つの矛盾する作用を発揮することが知られており、実際エンドトキシンショックモデルにおいて、誘導型NO合成酵素に対して選択性のないNO合成阻害剤を投与すると、エンドトキシンによる肝障害がむしろ増悪することが報告されている。そこで本年度は昨年度に引き続きNOの供給剤であるニトロソ化α_1-PIプロテアーゼインヒビター(ニトロソα_1-PI)を作製しその臓器保護作用について検討した。このため、ヒトの血漿中に多量に含まれ、血中の主要のプロテアーゼインヒビターとして知られるα_1-PIを、分離精製した後、α_1-PIを10等量(モル比)のisoamylnitriteで処理しニトロソα_1-PIを得た。このニトロソα_1-PIは、α_1-PIのSH基にNOを一個結合させたものであり、水溶液中でもNOを長時間保持していた。さらに、得られたニトロソα_1-PIは、ラット肝の虚血再還流障害モデルにおいて、著明な臓器保護作用を示し、この薬効はニトロソα_1-PIによる血流維持や好中球浸潤とアポトーシスの抑制作用によりもたらされているものと思われた。興味あることに、ニトロソα_1-PIは、α_1-PI本来の好中球エラスターゼ阻害作用に加えて、アポトーシスの発現に直接関与するカスパーゼの阻害活性や強い抗菌活性を有していた。以上の知見より、ニトロソα_1-PIの、各種ショック病態における多臓器不全に対する臨床応用の可能性が示唆された。
