質量分析によるグリコヘモグロビンの標準定量法の確立と実用化

研究概要	糖化ヘモグロビン(HbAlc)は重要な糖尿病の指標であるが、現行測定法では種々の要因による正或いは負の誤差を含んでおり、さらなる改良を要する。現在我が国では液体クロマトグラフィー(HPLC)法が80%を占め、残りが免疫法によって測定されている。個々の方法の再現性、方法間誤差は大きく、申請者らの検討では方法間誤差は20%に及ぶ。本研究は質量分析による測定法をモニターとして用い、誤差の極力小さい真値にできるだけ近い測定法を開発することにある。平成10年度の成果は昨年に引き続き、異常Hbの検出・同定を行い、また質量分析によるHbAlcの測定を行い現行法と比較した。異常Hbを含む検体は全例、従来法特にHPLC法では、血糖値を反映していない。α鎖異常では、HbAlc値がl/4低めになるにすぎないため、臨床症状の無い場合、異常Hbの存在に気づく事は困難であり、誤判断を招く可能性が高い。HbをV8プロテアーゼにより酵素消化し、β鎖アミノ末端の6アミノ酸からなるペプチドとそのN末にグルコース付加したペプチドの比率を質量分析によって求める方法が、HbAlcの標準法として提唱されている。申請者はこの方法の定量性をさらに高め、真値に近い値を求めるため、ペプチド研究所に注文して、高純度のHbアミノ末端のペプチド1-deoxyfructosyl-Val-His-Leu-Thr-Pro-Glu,Val-His-Leu-Thr-Pro-Gluを入手した。これを用い、次年度に向け研究を完成させる。また現行及び開発中のHPLCについて、HbAlc分画に混在する他成分を質量分析により調べた。従来、不安定Hb、アセチル、カルバミル、アセトアルデヒド等のHb結合物がHbAlc分画に含まれる事は既に報告されている。この他に申請者らは、ホルミルHb、カルボキシ末端アルギニン欠損Hbを同定した。
