胃癌の異分化に伴う遺伝子変化:肝様腺癌をモデルとして

研究概要

肝様腺癌は胃などに発生する肝細胞性形質をもつ癌腫でalpha-fetoprotein(AFP)を産生することが一般的である。本腫瘍を癌腫の異分化(transdifferentiation)のモデルとしてその発生や分化・表現形質を詳細に検討するのが本研究の目的である。これに伴って難治性消化器癌の一型としても知られている肝様腺癌の病態解明にも役立ち臨床的貢献もし得ると考えられるものである。平成9年度はmicrosatellite markerによる肝様腺癌の癌遺伝子変化検索として第4染色体長腕のある領域でのloss of heterogeneity(LOH)を調べたところ、4例中の1例においてLOHが確認された。この領域のLOHはAFPを産生する肝細胞癌(hepatocellular carcinoma,HCC)に特徴的であることが報告されているため、肝外に発生する肝様腺癌においてもHCCと共通した癌抑制遺伝子欠損などの発癌機構の存在が考えられた。また本年度は肝様腺癌における肝細胞性を分子遺伝的に解析したところ、肝様腺癌に特徴的な蛋白発現としてalbumin mRNAの大量発現、AFP及びtransferrin mRNAの発現が捉えられた。このことから肝様腺癌細胞の肝細胞性が明瞭に確認された。
