細菌感染によるHUS発症と予防に関する研究

研究概要

腹痛、食欲不振、血液の混入した水様性便に引き続きHUSを発症した女児の便より分離同定したBacteroides fragilis TDP101株の病原性を検討し、以下の点を明らかにした。
1.B.fragilis TDP101、YCH46(敗血症患者より分離)、ATCC25285の3株およびE.coli W3110株の36時間培養菌液(1.0-1.4×10^9cfu/ml)を2週齢の雄ウサギの小腸に注入し、48時間後に腸管内の液体貯留量を測定した。その結果、B.fragilis TDP101株では6.61mlの液体貯留があり、他のB.fragilis(2.3-3.2ml)やE.coliよりも高い値を示した。
2.また、生後2日目のICRマウスに上記菌液を経口投与し、3時間後の腸管内液体貯留量を測定したところ、B.fragilis TDP101株では他のB.fragilisやE.coliよりも高い値(p<0.01)を示した。
3.マウスにB.fragilis TDP101株を腹腔内投与したときの血中に出現する菌数はTDP101株で最も高い値を示したが、菌液投与の7日後には検出されなかった。
4.マウスにB.fragilis TDP株を腹腔内投与し、HUSを発症するかどうかを調べた。血液検査の結果、TDP101株投与群では貧血が認められ、血小板が減少していたが、腎機能不全は観察されなかった。
5.B.fragilis TDP101株、ATCC25285株、非病原性のB.uniformis1001株およびVero毒素産生菌E.coli KY1011株の無細胞抽出液を調製し、Vero細胞とHEL細胞に対する細胞毒性を調べた。その結果、B.fragilis TDP101株、ATCC25285株はVero細胞に対して640倍および160倍希釈まで細胞毒性を示し、HEL細胞に対しても160倍及び80倍希釈まで細胞毒性を示した。E.coli KY1011株の培養上清はどちらの細胞に対しても640倍希釈まで毒性を示したが、B.uniformisはどちらの細胞に対しても毒性を示さなかった。
