研究概要	抗リン脂質抗体症候群(APS)で見られる抗カルジオリピン抗体(aCL)は、カルジオリピン(CL)単体ではなくCLと血中のβ_2-グリコプロテインI(β_2-GPI)との複合体、正確にはβ_2-GPI上の抗原決定基を認識している。抗β_2-GPI抗体産生にはβ_2-GPI特異的T細胞が関与していると推定される。β_2-GPIのT細胞エピトープの解析を試み、以下の検討を行った。 1)β_2-GPIの特異的T細胞の検出 : APS患者リンパ球よりモノクローナル抗β_2-GPI抗体および抗プロトロンビン抗体産生株の樹立を試みた。 2)抗β_2-GPI抗体のB細胞エピトープの推定 : ランダムペプチドを用いたスクリーニングにより、抗β_2-GPI抗体のB細胞エピトープを推定し、β_2-GPIの3次元モデルの第4ドメイン上に、推定されたペプチドと同様の構造をとる部位を認めた。 3)β_2-GPI多型性とAPS : β_2-GPIの遺伝子多型性のうち、特に247Val/Leu変異では、抗β_2-GPI抗体の陽性率が247Valを持つ患者で高いことが示された。固相酸素抗体法による検討では、APS患者由来モノクローナル抗β_2-GPI抗体は247Valβ_2-GPIに対する結合性が247Leuβ_2-GPIより高い傾向が見られた。APS患者末梢血リンパ球を用いたT細胞刺激試験では、247Val_22-GPIに優位に反応する場合と247Leuβ_2-GPIに優位に反応する場合が見られた。 4)β_2-GPI欠損症の解析 : 正常人血中β_2-GPI濃度を測定し、欠損者2家系を発見した。欠損者ではβ_2-GPIの遺伝子の第4エクソンに欠落がみられた。欠損者の凝固系では非刺激時には明らかな異常は見られず、脂質系も2家系に共通した異常は見られなかった。ヘテロ欠損者でも血中β_2-GPI濃度の著明な低下がみられたが、脂質系に明らかな異常は見られなかった。
