炎症性腸疾患におけるマトリックスメタロプロテアーゼと潰瘍形成の関連

研究概要	炎症性腸疾患における細胞外基質分解酵素マトリックスメタロプロテアーゼ(MMP)の発現と病態との関連に注目し検討を行った。炎症性疾患全般についてみると慢性関節リウマチにおいて滑膜細胞がMMPを生産していることが既に明らかとされている。一方、炎症性腸疾患におけるプロテアーゼの役割は、これまで一部のセリンプロテアーゼについて報告があるのみで詳細な検討はなされていないが、炎症性腸疾患において炎症に起因する潰瘍形成などの組織破壊が認められることは明らかであり、この組織破壊に何らかのプロテアーゼが作用していることが考えられる。われわれはMMPが組織の破壊修復過程における重要な役割をもっていると考え、潰瘍性大腸炎における発現を検討した。免疫組織染色法を用いた検討でMMP-1,2,3,9については特徴的な染色傾向を認めないが、マトリライシンは高度な炎症所見を伴う潰瘍辺縁に強い発現を認め、潰瘍辺縁以外の上皮やリンパ球などの間質細胞には発現陰性の染色傾向を示した。また同病変でTIMP-1,2の発現は認められないことから、潰瘍辺縁部においてはマトリライシンが発現優勢であると考えられる。一方、中等度の炎症所見がある組織では、ほとんどが染色されず、RT-PCRでもマトリライシンの発現は認められない。マトリライシンの発現の認められる高度な炎症を伴った潰瘍辺縁の上皮細胞は、病理組織学的には再生像が強く、マトリライシンの発現と組織修復との関連が示唆された。
