肺損傷後のリモデリングにおけるシクロオキシゲナーゼ発現の分子生物学的解析

研究概要

肺の線維化は炎症に引き続く治癒過程とも考えられ、進行性の場合は予後不良の因子となる。このため、肺線維化の病態解析や治療法の開発は重要な研究課題の一つである。プロスタグランディン(PG)E2は線維芽細胞増殖やコラーゲン産性を抑制し線維化に対しては抑制因子の一つと考えられている。一方、肺線維芽細胞や肺胞マクロファージ/単球からのPGE2産生はTh細胞から産生されるサイトカインで調節を受けている。我々は従来より肺線維化とPGE2について検討を進めており、肺線維症患者の肺胞マクロファージのPEG2産生能は対照に比し低下していることを報告している。これらの知見から、PGE2産生調節の機序をTh1及びTh2サイトカインの関連から検討した。結果は以下の通りである。(1)肺胞マクロファージ・単球のPGE2産生はTh1サイトカインであるIFN-γでは影響を受けなかった。一方Th2サイトカインであるIL-4,IL-10,IL-13はPGE2産生を抑制した。Th2サイトカインはCOX-2発現を抑制しシクロオキシゲナーゼ活性を低下させた。この抑制の程度はIL-4>IL-10>IL-13の順であった。また、IL-4は抗炎症作用を有するアラキドン酸代謝物である15-HETEやLipoxinなどの律速酵素15-lipoxygenase産生を増強した。従来の報告を考慮するとTh2サイトカインは線維化の過程では抗炎症に作用するが線維化に対してはむしろ増強的に作用する可能性が示唆された。これらの結果は肺線維症の病態にTh1、Th2サイトカインによるPGE2産生調節が関与している可能性が示唆された。本研究で得られた知見は肺線維化の治療法開発に有用と考えられた。
