肺癌で欠失する癌抑制遺伝子産物を文し標的とした遺伝子治療

研究概要	癌性胸膜炎患者7名より胸水を採取し、癌抑制遺伝子の発現状態を検討し、胸水中有核細胞を用いたコロニー形成試験で各種遺伝子の導入効果を検討した。男性5名、女性2名で、年齢の中央値は72歳であった。組織学的には腺癌6名、小細胞癌1名であった。有核細胞20万個当たりのコロニー形成数は0より120個で、7名中2名はコロニー形成がみられず、遺伝子治療の効果判定不能であった。pcDNAp16(p16cDNAの全長を含む)またはpRC/CMVRb(rb cDNAの全長を含む)をリポソームを用いて導入をした。遺伝子導入2週後にコロニー数で判定すると評価可能5名中、p16では2名で、rbでは1名でコロニー数の有意の減少がみられた。免疫組織染色でp16陽性やrb陽性とp16、rb遺伝子治療効果との間に相関性はみられなかった。抗癌剤との併用効果の検討では、シスプラチンとp16併用でコロニー数の減少において相乗効果がみとめられたが、5-FUやビンデシンとの併用では相乗効果はみられなかった。コロニー形成試験の結果、癌性胸膜炎に対する遺伝子化学療法はp16とシスプラチンの組合せが最も適切と考えられた。
