研究概要

「非アルツハイマー型変性疾患における痴呆の発現機序に関する研究」のうち、2年度は進行性核上性麻痺(PSP)とびまん性レビー小体病(DLBD)に関する研究を行った。PSPでは皮質下核のみならず海馬を含む大脳皮質にも広範にタウ陽性細胞骨格異常を示す神経細胞が出現し、PSPの痴呆の発現機序と密接に関連すると考えられ、また皮質基底核変性症(CBD)との異同が問題となっている。 今回の研究では、各種の抗タウ抗体を用いた免疫組織化学によりPSP剖検例におけるタウ陽性神経細胞の出現量と分布および抗原性を定量的に検討した結果、タウ陽性神経細胞には神経原線維変化(NFT)を有するものと線維形成を欠くものとがあり、両者の出現程度によりPSPは典型例とCBDに近い二群に分かれること、また大脳皮質にみられるNFTはアルツハイマー病(AD)-NFTの好発部位であっても多くはPSP-NFTであることが示された。また、ユビキチン免疫組織化学によりDLBDの海馬におけるユビキチン陽性構造物の多くが記憶障害と密接に関連するperforant pathwayの線維末端の変性よりなることを前年度に示した。 今回の研究では、DLBD剖検例の海馬におけるα-シヌクレイン免疫組織化学によりこれらはレビー小体(LB)と同様にα-シヌクレイン陽性であり、免疫電顕的にもLBと同様の微細構造物を有しており、perforant pathwayの起始細胞である嗅内野皮質のpre-αには少数のLBしかみられないことから、海馬内α-シヌクレイン構造物はperforant pathwayの“dying back"変性によってα-シヌクレインがニューロフィラメント等に蓄積して形成されることが示唆された。
