白血病及び正常造血幹細胞におけるWT1遺伝子の発現様態の解析

研究概要	全ての白血病で、Wilms腫瘍の原因遺伝子であるWT1遺伝子が異常高発現しており、RT-PCR法でこれを定量することにより、微小残存白血病(MRD)の測定が可能である。一方、正常造血幹細胞分画をCD34,CD33,CD38などでFACSsortingして、幹細胞分画におけるWT1遺伝子発現を調べたところ、CD34^+CD33^-分画にのみ、発現が認められた。しかし、白血病細胞における発現より10倍以上低いものであった。白血病再発の早期診断を行うには、この正常造血幹細胞亜分画における発現は、骨髄でのMRD検出の感度を下げることになる。そこで、コントロールとして、WT1を発現しない悪性リンパ腫の患者で、同種骨髄移植後の幹細胞分画のFACSでの解析とWT1遺伝子発現の定量を、経時的に同一検体で行ったところ、移植後4〜5カ月間、CD34分画の減少が観察され、それに伴ってWT1値が1オーダー以上低下することが判明した。一方、白血病患者では、再発時は白血病細胞当たりのWT1発現量が上昇することが観察されているので、両者を合わせると、移植後の再発早期診断は、従来考えられていたよりも、より早期に行えることが判明した。これらのデータを基に、骨髄移植を受け、その後再発を来した6人の白血病患者で、寛解状態より再発ヘのステップに至るturningpointを解析したところ、中央値49日(22-104日)であり、それを過ぎると、指数関数的に腫瘍量が増加していくことが判明した。したがって、骨髄移植後の再発へのfirststepは移植後極めて早期に生じており、移植後の免疫抑制剤の調節を移植早期より行う必要があると考えられた。
