虚血性神経組織半影(penumbra)のin vivo系におけるモデル化

研究概要	本研究の目的は、虚血性神経半影(Penumbra)のモデル化とその応用である。我々はこれまでin vitro系及びinvivo Ishemic Penumbraモデル化に成功した。そして、Penumbraにおける脳血流(CBF)と組織酸素飽和度との関係を検討してきた。その結果、虚血性脱分極が生じた時点ではCBFの低下に比し酸素飽和度は保たれていたが、これは酸素摂取率の増加を反映していると推察された。今回はこのモデルを用いて、神経毒3-Nitropropionic acid(3-NPA)を用いてchemical preconditioningを発現させ、preconditioningのpenumbraに対する影響を検討した。雄Wistar ratを生理食塩水投与control群(A群;n=8)とchemical preconditioningを起こすと考えられている3-NPA(20mg/kg)腹腔内虚血3時間前投与群(B群;n=10)の2群に分け、in vivo Ishemic Penumbraモデルを用いて脱分極が生じるまでの時間を比較した。脱分極が生じた時点でのrCBFはA群、B群ともに平坦脳波時のrCBFに比し有意に低下していたが、酸素飽和度は有意差は認めなかった。また、平坦脳波時のrCBFおよび酸素飽和度は、A群では11.0±3.9LD-units・29.8%、B群では8.0±2.2LD-units・30.6%、脱分極が生じた時点では、A群では5.2LD-units・24.9%、B群では5.4LD-units・24.6%と両群間に有意差は認めなかった。脳波が平坦となってから脱分極が生じるまでの時間は、A群89.4±20.2秒、B群158.0±66.8秒であり有意差が認められた。以上より、3-NPAによるchemical preconditioningにより、虚血負荷時に神経細胞の電気的活動が静止してから脱分極が生じるまでの時間は延長されるが、CBFおよび酸素飽和度には影響しなかった。本モデルではin vivoにおいてpenumbraの状態をとらえることが可能であり、今後penumbraの病態解明を行う上で有用なモデルに成り得ると考えられた。
