経頭蓋電気刺激誘発脊髄電位と誘発複合筋活動電位による脊髄運動機能回復の予後予測

研究概要	ヒトの術中運動機能モニタリングにおいて、ECMAPが脊髄損傷の指標として適切かは不明である。本研究では、脊髄急性圧迫に対しECMAPの変化を記録し、術中運動機能モニタリングとしての有用性について検討した。10匹の成猫を用い、ケタミン麻酔下に開頭し皮質運動野を電気刺激した。撓側手根屈筋からECMAP(誘発複合筋活動電位)を、C6硬膜外からESCP(誘発脊髄活動電位)を記録した。まず、5匹に刺激頻度と刺激回数を変えて刺激し、最大振幅のECMAPが得られる刺激条件を求めた。様々な組み合わせで刺激を行ったところ、500Hzで5回の連続電気刺激により最大振幅のECMAPが記録されることが解った。これを至適刺激条件と考え、以下の実験は全てこの条件で行った。脊髄圧迫下でECMAPおよびESCPが如何に変化するか、また圧迫後の運動機能の関連性を検討するため、5匹の成猫でDynamometerを用い脊髄圧迫モデルを作製した。段階的圧迫を加えたところ、圧迫の増加に伴いECMAPの電位振幅は低下を示し、潜時は延長した。30gの圧迫でECMAPは全例消失した。ESCPも圧迫とともに電位振幅は低下し潜時は延長した。ECMAPが消失してもESCPの電位振幅がコントロールの60%以下になる例はなかった。手術後3匹は覚醒させ3週後に運動機能の評価を行ったところ、今回の例では運動機能障害を残すものは認められなかった。以上より、術中脊髄運動機能モニタリングとしてのECMAPはESCPに比較し鋭敏な脊髄障害の指標になりうることが解った。しかし、鋭敏すぎるがゆえ脊髄損傷の予後判定の指標にはなり難たく、現時点ではSEPなどの従来のモニタリング法との併用が望ましいと考えられた。
